(U) Executive Summary (U//LE3) On 15 April 2021 at approximately 11:04 pm (EST), an active shooter incident occurred at FedEx Facility, 8951 Mirabel Rd., Indianapolis, Indiana, which resulted in eight casualties and five wounded. In addition, the shooter, Brandon Scott Hole, died from a self- inflicted gunshot wound b7E when FBI Indianapolis Joint Terrorism Task Force (IP JTTF) initiated a Guardian assessment, through 15 April 2021, when the active shooter incident occurred..
