1 consider the facts stated on the application of the applicant,

3 . (1) need not consider whether or not the alleged
4 assailant has been apprehended or brought to trial, nor
5 the result of any criminal proceedings against that
6 person;
7 (2) shail determine the amount of the ‘pecuniary
8 loss to the applicant and, in the case of a deceased vic-
9 tim; of other persons dependent on the victim for their
to ‘support, basing this determination on the definition in
i section 6; o
12 (3) shall determine the degree or extent to which
18 the victim’s acts or conduct provoked or contributed
44, — to his injuries or death and reduce or deny the award
15 of compensation accordingly;
46 (4) shall deduct the amount of benefits, payments
wv or awards, payable under the District of Columbia Un-
18 employment Compensation Act, the District of Colum-

20 ability Act, or from any other local governmental or
21. Federal funds, which the applicant or other person’
22 dependent for his support on a deceased victim, as the
23 case may be, has-received or to which he is entitled as a
24 result of the injury to or death of the victim; and

A oath, on a form prescribed and furnished by the Commission,
2 setting out—

(1) the name and address of the victim;
{2} if the victim is deceased, the name and address
of the applicant and his relationship to the -victim, the
names and addresses of other persons dependent on the
victim for their support and the extent to which each
is so dependent;
(3) the date and nature of the crime or attempted
crime of violence on which the application for compensa-
tion is based; ,
(4) the date and place where and the law enfor¢e-
ment official to whom notification of the crime was given;
(5) the nature and extent of the injuries sus-
tained by the victim, the names and addresses of ‘those
giving medical and hospitalization treatment to the vic-
tim and ‘whether death resulted;
(6) the pecuniary loss to the applicant and to such
other ‘persons as are specified under ‘paragraph (2)
resulting from the injury or death; ; (
(7) the amount of benefits, payments or awards,
if any, payable under the District of Columbia Unem-
ployment Compensation Act, the District of Columbia
Policemen and Firemen’s Retirement and Disability Act,
or from any other local governmental or Federal funds,
