In order to receive any information other than public source material, you must provide a which may or may not be contained in our records. If you want us to search for any releasable public source information, please let us know in writing. In addition, to ensure an accurate search of our indices, please provide the complete name, as well as the date and place of birth of the subject(s) if you have not already done so. Absent a privacy waiver or proof of death, the disclosure of law enforcement records about another person is considered an unwarranted invasion of personal privacy. Such records are exempt from disclosure pursuant to exemptions (b)(6) and/or (b)(7)(C) of the FOIA, Title 5, United States Code, Section 552 Proof of death can be a copy of a death certificate, obituary or recognized reference source. Privacy waivers must be directed specifically to the FBI, and they must be notarized or include a certification of identity. You may submit an appeal from any denial contained herein by writing to the Co-Director, Office of Information and Privacy, Department of Justice, Suite 570, Flag Building, Washington, D.C. 20530, within 60 days from receipt of this letter.
